Job Description: The Primary Health Counsellor reports directly to the Primary Health Care Manager. Professional peer review and responsibility will be maintained through a joint accountability to Mental Health/Addictions. The Primary Health Counsellor is an integral member of the primary health care team, in a collaborative, co-location model. As a member of a multidisciplinary primary health care team, the incumbent is primarily responsible for supporting the providers in PHC services in screening, brief intervention and accessing appropriate services for ongoing mental health and psychosocial services for their clients. The services to be delivered through this position include screening and assessing the needs of referred clients and determine a relevant treatment plan which may include short term intervention with the counselor and/or case management or referral to other community programs. The Primary Health Counsellor initiates and/or participates in community development initiatives and participates in the evaluation of PHC initiatives within the site. He/she promotes quality health care outcomes working within established standards of practice to ensure that clients receive a high standard of primary health care.
