Job Description: Reporting to the manager of the COPD program, the nurse clinician: has primary clinical responsibility for the chronic obstructive pulmonary disease (COPD) provincial program. Utilizes established criteria to assess and plan each client's self-management care needs, providing appropriate intervention to ensure timely access to care. Assesses client needs/concerns through chart review, history taking, physical assessments, and lab data and in consultation with physicians and multidisciplinary team members. Liaises with multidisciplinary team members, hospital departments, and community agencies to advocate for continuity and coordination of care. Provides efficient and effective self-management education/counselling for clients/families/caregivers in all aspects of copd management, care and treatment. Provides continuing health professional education through communication, resource materials, presentations and in-service education. Assists in the development, monitoring and evaluation of programs, protocols, guidelines and clinical standards of practice, in collaboration with the manager, physicians and multidisciplinary team members. Participates in program administration and supports research, program development, planning, quality monitoring and evaluation. Inputs and maintains a demographic and clinical web based database for the COPD program. The nurse clinician will provide care within a holistic and culturally appropriate framework for the population served with an understanding of the health equity and the impact of poverty and risk conditions on health.

Experience

  • 5 years of clinical experience related to patients with COPD required. A minimum of 1 year experience in patient/family/professional
    education is required. A minimum of 1 year experience in program/resource development related to needs assessment planning,
    program design, implementation and evaluation, teaching/counseling, facilitation skills and leading group activities is required. -
    Experience in teaching staff and patients in one to one and group settings

Education

  • Bachelor Degree/Diploma in Nursing

License

  •    Basic Life Support (BLS)
    • Must possess a valid Saskatchewan driver's license and have access to a reliable vehicle
    • Registered or eligible for registration with the College of Registered Nurses of Saskatchewan (CRNS)

Other Education and Training

  • Nursing diploma is acceptable
  • Basic Life Support Certificate (BLS)

PROGRAM WITHIN THE ENTIRE HEALTH SYSTEM. KNOWLEDGE OF THE PRINCIPLES OF ADULT EDUCATION. REQUIRES EXPERTISE IN
DESIGN, DEVELOPMENT AND EVALUATION OF SPECIALITY PROGRAMS. RESP TREC CERTIFICATION REQUIRED WITHIN 6 MONTHS OF
START DATE. MASTERS DEGREE IN RELATED FIELD IS PREFERRED. The COPD Program is ran out of RUH and SPH, this positions primary
location will be SPH’s. Intermittent travel to RUH will be required. The Saskatchewan Health Authority (SHA) requires a criminal
record check (CRC) from every potential SHA team member. The CRC must be dated within the past six (6) months and be satisfactory to
the SHA. A vulnerable sector check (VSC) is required for individuals providing patient/resident/client care to vulnerable persons. Internal
