Job Description: Reporting to the Director of Continuing Care, the Long Term Care Community Nurse is an integral member of the Community Based Long Term care team, providing comprehensive nursing assessments and coordinating community services to patients in the Community Long Term Care Program. The Nurse promotes quality health care outcomes working within established standards for case management practice. The Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Community Based Long Term Care initiatives.

Experience

• Min. 2 yrs of recent nursing exp. in a long term care/ community based setting 

Education

• Bachelor of Science in Nursing (BScN)

Licenses

• Basic Life Support Certificate (BLS)

Other Education and Training

• Nursing diploma is acceptable

Competencies

• Case management skills
