Job Description: THE LIVEWELL DIABETES PROGRAM AIM 4 HEALTH IS AN INTERDISCIPLINARY DIABETES CARE TEAM THAT OFFERS DIABETES PREVENTION, DETECTION, CARE, AND TREATMENT SERVICES TO A DIVERSE POPULATION, WITH A FOCUS ON THE NEEDS OF FIRST NATIONS, MTIS & IMMIGRANT PEOPLES. PROGRAMS ARE DELIVERED IN A HOLISTIC AND CULTURALLY APPROPRIATE MANNER AND IN A VARIETY OF WAYS SUCH AS CENTRE-BASED AND OUTREACH INTO THE COMMUNITY IN PARTNERSHIP WITH COMMUNITY ORGANIZATIONS. REPORTING TO THE MANAGER OF CHRONIC DISEASE MANAGE
