Profile Description:
The Social Worker, BSW has responsibility for psychosocial assessment and intervention, complex discharge planning and supportive counseling for patients with sub-acute and chronic health conditions, relating specifically to rehabilitation and living with disability. The patient group is primarily adults with neurological diagnoses. The social worker offers a professional working relationship, including an understanding of the social work role, and SHA philosophy, objectives and policies. The social worker, BSW exercises a range of intervention skills related to number of theoretical models. The Rehab program involves services of a large interdisciplinary team of which social work is an integral member. Receives clinical supervision and administrative direction from the Manager and/or Senior Social Worker.
