Job Description: Reporting to the Program Manager - Saskatchewan Donation and Transplant Program, and as part of the interdisciplinary team, the Social Worker is responsible for the full scope of case management services in the donation and transplant program. The core functions of case management practice include referral/intake, engagement, assessment, care planning, monitoring, evaluating, donor aftercare and termination/discharge. This is done through collaboration with the patient, family, care givers and other members of the multi-disciplinary team. The Social Worker liaises with other services in the Regina area, throughout the province and interprovincial, in an endeavor to provide the right service at the right time and in the right place. The incumbent assesses and provides counseling to clients and their families dealing with the complexities of donation and transplantation. The Social Worker will function as a member of the Saskatchewan Donation and Transplant Program with an emphasis on the team approach to care.

Human Resources Exemption: No

Experience

  • One year of experience in an acute care health setting is required
  • Experience in transplantation, crisis intervention, and grief work is an asset

Education

  • Bachelor of Social Work (BSW)

Knowledge and Abilities

  • Demonstrated supervisory skills and application of judgment
  • Ability to develop quality monitoring program to assess/improve quality of care
  • Knowledge and ability in various counseling modalities
  • Ability to critically evaluate social work functions to refine services
  • Demonstrated ability to integrate research based theory into practice
  • Knowledge in individual assessment and counselling skills
  • Can conceptualize/chart issues patient/family face to engage multi-discipline team

  • Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement.  HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.
