Hiring Rate: $50.62 - $67.73
Hours of Work: Primarily Monday-Friday Days, Afternoons and Weekends as required. (Subject to change).
Reporting Relationship: Director, Pharmacy Services.
We are seeking a highly skilled Drug Use Evaluation (DUE) Pharmacist to lead medication use evaluations, formulary management, and pharmacy quality improvement initiatives. This role is ideal for a pharmacist with strong analytical, clinical, and collaboration skills who is passionate about optimizing medication use across the hospital.
Key Responsibilities :
Formulary Management & Drug Information
- Maintain and update the hospital formulary, drug use and parenteral manuals, ensuring adherence to policies and best practices.
- Communicate and distribute timely updates on formulary changes, medication shortages, guidelines, and policies to hospital staff through appropriate channels.
Medication Utilization & Evaluation
- Analyze medication utilization trends and expenses by therapeutic classification to identify areas for improvement.
- Conduct retrospective and prospective drug utilization evaluations (DUEs), ensuring cost-effectiveness and clinical appropriateness.
- Manage medication shortages in collaboration with the pharmacy inventory team, ensuring timely identification of alternatives and communication.
Collaboration & Committee Participation
- Work closely with the Pharmacy & Therapeutics (P&T) Committee, physician leaders, and clinical pharmacists to develop and implement drug use policies.
- Participate in key hospital committees such as Safe Medication Practice and Pharmacy Sustainability.
Technology & Data Management
- Utilize pharmacy software, data analytics tools, and reporting systems to track medication trends and savings.
- Work with inventory staff to optimize medication selection and distribution strategies.
Quality & Patient Safety Initiatives
- Lead adverse drug reaction (ADR) reporting initiatives, ensuring timely reporting to P&T and external bodies where appropriate.
- Support continuous quality improvement (CQI) with a focus on medication safety and efficiency.
Requirements: (Skills & Qualifications)
Education & Licensure
- Licensed Pharmacist (Part A) with the Ontario College of Pharmacists (OCP) - required.
- Advanced degree in Clinical Pharmacy or completion of a hospital residency program - preferred.
Experience & Technical Skills
- Minimum 5 years of hospital pharmacy experience with expertise in clinical pharmacotherapy and pharmacoeconomics required.
- Experience with drug utilization evaluation, formulary management, and policy development.
- Demonstrated experience working with corporate hospital committees to implement medication use policies and quality initiatives.
Core Competencies
- Strong problem-solving, critical thinking, and analytical skills.
- Excellent written and verbal communication skills, including presentations and education.
- Ability to work independently and collaboratively within interdisciplinary teams.
- Proficiency with pharmacy software, data management tools, and Microsoft Office (Excel, Word, PowerPoint)
- Excellent attendance and discipline free record required.
