Join our team of Allied Health Professionals as we work together to deliver compassionate care that is responsive to the healthcare needs of our community. Currently, we are looking for a Social Worker to work collaboratively with our Geriatric Outreach Team and Ambulatory Medical Clinics.

As part of our Allied Health department of interprofessionals, Social Workers assist our patients by providing counselling and support, as well as, assisting patients in accessing community resources. Reporting Relationship: Manager of Allied Health and Ambulatory Medical Clinics

Hourly Rate Range:  $44.67 - $57.90 

Schedule Requirements Available to work rotating shifts (Monday to Friday, Weekends and Statutory Holidays). Current schedule is Monday to Friday, which is subject to change.  

Location: Primary location is Wilson Site. Home visits are required for this position, the successful candidate must have a vehicle for use.

 

Position Responsibilities:

  • Maintains an awareness of community resources, recommend strategies to bridge the identified gaps and assist patients in establishing linkages with community resources
  • Works in a manner to ensure safety to oneself, clients, colleagues and the work environment.
  • Discharge planning and patient flow within unit and community sectors
  • Act as a patient and family advocate, ensuring their rights, needs, hopes and values are recognized
  • Flexibility in caseload and program assignment.
  • Act as a resource to hospital staff and the inter-professional team in assessing, planning and managing the psychosocial care of patients
  • Participates in reflective practice as per identified college standards
  • Provides cross-coverage of in-patient Social Work Services as needed.
  • Geriatric Outreach Team Social Worker responsibilities:
  • Conducts comprehensive, in-home geriatric assessments to address: the referral source identified issues and concerns and formulate an appropriate plan of treatment in consultations with the client, caregiver and primary care physician
  • Provides feedback to referral source, primary care physician and community agencies regarding the outcome and recommendations
  • Participates in the interdisciplinary approach to geriatric care and ongoing learning in the area of geriatric care and elder care management
  • Ambulatory Medical Clinics Social Worker responsibilities:
  • Perform psychosocial assessments on outpatients in the ambulatory environment to identify gaps in access to the appropriate resources that are required to self-manage their health in the community
  • Assess patients and family members to determine psychosocial needs, strengths, limitations and barriers to treatment and care
  • Provide crisis intervention in collaboration with team
  • Assess the impact of social determinants of health on the wellbeing of patients and their families, by utilizing an anti-oppressive framework
  • Counsel patients and families to help them clarify and meet their goals and provides services to aid in their attainment
  • Inform patients and families of appropriate community resources and assists in their coordination

Qualifications:

  • MSW obtained from an accredited school of Social Work required.
  • Current membership with OASW and OCCSWSSW, or eligibility to become a member required.
  • Experience in the acute and/or sub-acute sector system, patient flow and discharge planning is an asset and preferred.
  • 2 to 3 years experience within an acute hospital setting is an asset and preferred.
  • Computer skills required; ability to manage electronic recording, workload and meet submission timelines.
  • Demonstrated knowledge of the acute, sub-acute and community sector. Knowledge of community resources an asset.
  • Demonstrated organizational skills to set clinical and non-clinical priorities in a fast paced environment.
  • Demonstrated ability to work independently and in a team environment.
  • Demonstrated excellence in interpersonal skills.
  • Excellent communication skills (oral and written) facilitating effective communication in case reporting and assessments, with patients, families, peers, other healthcare team members and volunteers.
  • Excellent decision making skills.
  • Demonstrates cross cultural awareness and sensitivity.
  • Excellent attendance and a discipline free record are a requirement.
