Community Stroke Rehabilitation Team

POSITION SUMMARY:
A challenging exciting opportunity exists to be a member of a collaborative inter-professional team delivering an innovative rehabilitation service to help stroke survivors and their caregivers reach their optimal level of recovery in their own community.
Using the principles of person-centered care, the occupational therapist is responsible for providing services to referred stroke clients in a variety of settings, with a focus on cognition, perception, community integration and attaining optimal recovery in the areas of self-care, productivity and leisure. As well, the occupational therapist will ensure optimal independence and safety in the home and return to or development of new activities and roles that are important to the client. They will also provide client-specific consultation and education/training/in-services related to stroke rehabilitation for service providers, caregivers and families.
REPORTS TO: Manager, District Stroke Centre & Dialysis
DATE REQUIRED: As soon as possible
SALARY: $40.36 to $56.30 per hour
QUALIFICATIONS:
Education:
- Master’s degree or equivalent from a recognized accredited university program in Occupational Therapy
- Registration in good standing with the College of Occupational Therapists of Ontario
- Eligibility for membership in the Canadian Association of Occupational Therapists (CAOT) or Ontario Society of Occupational Therapists (OSOT)
- Montreal Cognitive Assessment (MoCA) training and certification required
- Current Basic Life Support certification required
- Completion of Apex Innovations® Canadian Hemispheres preferred
- Aphasia Institute’s Supported Conversation for Adults with Aphasia (SCA™) preferred
- Certification in Non-violent Crisis Intervention training preferred
- A valid Ontario driver’s license
Experience:
- Clinical expertise and experience in stroke rehabilitation
- Demonstrated knowledge of current Canadian Stroke Best Practices
- Experience in both hospital and community service delivery is an asset
- Knowledge of and recent clinical experience within the Huron Perth healthcare system preferred
Abilities:
- Demonstrated ability to practice effectively in an inter-professional collaborative environment
- Understanding of the role of regulated and unregulated healthcare professionals in the provision of stroke rehabilitation
- Demonstrated knowledge and clinical experience in neurological rehabilitation
- Working knowledge of and experience with theories and principles guiding adult learning and patient self-management approaches
- Demonstrated commitment to ongoing professional development
- Proven flexibility and a high level of initiative
- Motivated team player with excellent organizational and planning, leadership, critical thinking, problem-solving, decision-making and time management skills
- Able to perform responsibilities with minimal supervision
- Excellent English verbal and written communication skills using different mediums, such as telephone, in-person and video
- Client, family and caregiver service-oriented, able to work effectively with diverse populations/groups
- Well-developed interpersonal skills with a demonstrated ability to establish positive relationships with clients, families, caregivers, colleagues and community services
- Demonstrated ability to prioritize and function both independently and collaboratively within an inter-professional care team
- Excellent assessment skills and development of personalized patient informed treatment plans
- Able to support clients in their homes or in a community environment
- An understanding and appreciation of rural communities
- Evidence of continued professional growth and/or professional development
- Proficiency with computer applications including Microsoft applications and working knowledge of or the ability to quickly comprehend the Meditech Information System
