Position Summary
Interior Health has an opportunity for a relief part time (0.59 FTE) Case Coordinator Acquired Brain Injury (Neuro Rehabilitation) to join our team in Kamloops, B.C. This relief position is until the return of the incumbent.

What Will You Work On?


This position, in consultation with the client, family, and interdisciplinary team members, provides:

1. Case coordination to initiate and develop a plan of care, client and family counsel on issues that have impact on their care, referrals to other community resources as appropriate, and evaluation of care plans.
2. Individualized assessments and treatments for clients with complex neurological, traumatic and rehabilitative conditions into a practical continuum of care.
3. Coordination and linkage between acute care and community services such as the Brain Injury Association, ICBC, private practitioners and rehabilitation programs, to facilitate effective discharge planning for clients with Acquired Brain Injury (ABI) and other complex neurological/rehabilitation conditions.
4. Support as required to the ABI steering committee and/or ABI leadership group.

Qualfications
Education, Training and Experience:

• Bachelor’s Degree in a Health or Behavioral Science related field from a recognized university with additional educational preparation in acquired brain injuries or equivalent combination of education training and experience in the area of brain injury.

• A minimum of three (3) years Clinical experience in community-based and institutionally based brain injury rehabilitation programs or equivalent experience working with specialized populations in the community.

• Experience working with differing populations such as families in crisis or after trauma.

• Community based experience working with adults with ABI.

• Current practicing membership in appropriate licensing body.

• Valid drivers license
