An exciting permanent full time Community Health Facilitator opportunity is available in Interior Health’s Healthy Communities Initiative. This position has a flexible work location within the Kootenay Boundary and Central Kootenays.

What will you work on?

The Healthy Communities Initiative aims to reduce the burden of chronic disease by working collaboratively with local governments in creating supportive environments for health. Key focus areas are: Healthy Eating, Physical Activity, Tobacco Reduction, Healthy Built Environment, and Priority Populations.

The Community Health Facilitator works with other Promotion and Prevention, Community Integrated Health Services and Interior Health staff to build multi-sector partnership structures to support baseline community assessments, community engagement and healthy community action planning. The Community Health Facilitator supports local governments and other community partners in the integration of health promotion and disease prevention evidence/best practice, and researches information into effective and efficient policy and practice, including the development of healthy living strategic plans.

The position of Community Health Facilitator offers:

• Supporting cross-portfolio capacity for collaborative work with local communities.
• Facilitating collaborative multi-sectoral partnerships.
• Supporting local governments and communities to mobilize around healthy community initiatives aimed at addressing risk factors for chronic disease.
• Identifying a baseline of current local actions that promote health.
• Facilitating the development of future actions.
• Assisting local government to access and interpret health data.
• Illustrating relevant evidence-based strategies based on the five pillars of BC Healthy Communities for improving specific chronic disease risk factors.
• Supporting communities in program/action evaluation planning.
• Ensuring good relationships are developed and sustained with local governments and stakeholders.

Scheduling Information:

Shift types are days and shift times are from 08:30 to 16:30 hours. Days of work are Monday to Friday.

Qualfications
Education, Training & Experience:

Bachelor’s Degree in health, social sciences or management with content in health promotion, community development, population health and/or community capacity building.

Minimum of two years recent relevant experience (including experience with program evaluation), with a focus on facilitating integrated approaches to community health and well-being.

Current valid BC Driver’s License.

**As travel is involved in this role, a valid Driver’s License (class V) is necessary and access to a personal vehicle may be required.
