Position Summary
New opportunity for a Registered Social Worker to join the East Kootenay Primary Care Network team in Creston, B.C. This is a permanent full-time opportunity. This position works Monday to Friday from 08:30-16:30 with weekends and Stat holidays off.

About the position:
You will provide comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, counseling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.
Primary Care Networks are community-based networks of health care professionals and clinics who plan and deliver all day-to-day health care needs of a community. Within these networks teams of providers work together closely. In this is way, patients receive better care, by the right person, at the right time, closer to home.

The quiet community of Creston is located south of Kootenay Lake in the heart of the Rockies Region in Southeast BC and is nestled between the Purcell and Selkirk Mountains. Creston boasts many local Artisans with skills such as blacksmithing, pottery, quilting, broom-making and painting and display their creations at annual events such as the Arts and Culture Festival and Art Walk. If you are seeking to relocate, Creston has a complete range of services and facilities, which provide for a comfortable and satisfying lifestyle. Known as “The Valley of the Swans,” the Creston Valley is a breathtaking combination of snow-capped mountain peaks, fields of grain, deep clear lakes and wide-open spaces. Qualfications
Bachelor’s Degree in Social Work from an accredited university
Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
Current full registration with the British Columbia College of Social Workers.
Current valid BC driver’s license.
