Position Summary
Joining us at Williams Lake Urgent and Primary Care Centre, you’ll play a pivotal role within a coordinated approach to health care delivery ensuring clients receive the right care by the right provider, when and where they need it.
This position is eligible for a $20,000 recruitment incentive plus relocation allowance. Earn $42.27 to $52.68 per hour.

About the job:
As a key member of medical based team, the Social Worker provides comprehensive assessment, care planning, and treatment coordination for urgent and primary care clients and their families. Collaborating closely with other healthcare providers, the Social Worker delivers direct psychosocial care, counseling, group work, and system navigation. They advocate for clients, educate families, and connect individuals to specialized complex care resources, all while empowering clients to manage their own health and well-being.
The Social Worker is dedicated to enhancing the social and emotional functioning of the identified population by developing and delivering services that maintain or improve their health and overall quality of life. With a focus on client-centered care, the Social Worker plays a pivotal role in fostering positive outcomes for individuals and their families. Days of work are Monday to Friday; 10:00 to 18:00.
About Williams Lake UPCC
The UPCC will provide same-day care for people who need access to primary care within 12 to 24 hours, but do not require an emergency department. Conditions such as sprains, cuts, fevers and minor infections are appropriate for the UPCC. People can access the centre’s primary care via in-person, walk-in same-day appointments. The UPCC will be available to people who do not have a primary care provider, as well as people with a provider who are unable to schedule an appointment within 12 to 24 hours. Health-care providers will include family physicians, nurse practitioners, nurses, social workers, physiotherapists, Indigenous patient navigators and administrative staff.

Qualfications
- Bachelor’s Degree in Social Work from an accredited university.
- Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
- Current full registration with the British Columbia College of Social Workers.
- Current valid BC driver’s license.
