Position Summary
We have a relief (temporary) 0.5fte Registered Social Worker opening at the Urgent and Primary Care Clinic (UPCC) in Penticton. This position works a combination of day and evening shifts from 11:30-18:30 and 16:00-20:30. The position works a rotating schedule of 5 days on, and 5 days off, followed by 2 days on and 2 days off. The UPCC is staffed by a dynamic multidisciplinary team of Physicians, Nurse Practitioners, Nursing, Physiotherapy, MHSU clinicians and administrative support.

About the position:

The Urgent Primary Care Social Worker provides comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, counseling and group work, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management. The Social Worker is responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

Qualfications
Bachelor’s Degree in Social Work from an accredited university.
Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
Current full registration with the British Columbia College of Social Workers.
Current valid BC driver’s license (minimum class 7N)
