Position Summary

Salary Range:

Effective April 1, 2024, the pay scale for this position is $42.27 to $52.81

What Will You Work On?

This position is connected to the new Integrated Health Outreach Team. This team will offer low barrier, mobile primary care services to individuals in temporary housing situations, with specific focus on the HEARTH sites.

In this role, you will provide comprehensive assessment, care planning and treatment coordination for designated clients and their families. Working collaboratively with other health care providers and the multidisciplinary team, you will provide direct psychosocial care, counseling, system navigation and advocacy, education and linkage to relevant specialized complex care resources while facilitating client self-management. As the Social Worker you will be responsible for providing and developing social work services to maintain or improve the health and social well-being and functioning of the identified population.

Scheduling Information:

Shift types are days and evenings and shift times are from 09:00 to 17:00 hours. Social Work rotation is 4 on, 4 off and includes work on weekends and stats.

Qualfications
Education, Training and Experience:

Bachelor’s Degree in Social Work from an accredited university.

Two years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.

Current full registration with the British Columbia College of Social Workers.

Current valid BC driver’s license.
