Description
POSITION SUMMARY
The Clinical Practice Leader (CPL), Diabetes works with the leadership team within the clinical area and is responsible for provision of visible clinical support, coaching and modeling to ensure that the care provided in the hospital setting meets current professional and health care standards. He/she will work in collaboration with the interprofessional team to facilitate optimal patient care through education, providing resources and protocol development in a way that is consistent with Mackenzie Health’s vision, mission and strategic direction. Serves as a bridge between the inpatient/acute care setting and the outpatient Diabetes Education Centre.
QUALIFICATIONS
- Registered Nurse in good standing with the College of nurses of Ontario required.
- Baccalaureate degree in Nursing required - Masters degree preferred.
- Currently certified with the Canadian Diabetes Educator Certification Board (CDE) required.
- Current Basic Cardiac Life Support required.
- Minimum of five (5) years of relevant (diabetes) experience in clinical practice
- Minimum of two (2) years of progressive leadership experience
- Member of the Diabetes Canada preferred.
- Professional Association Membership preferred.
- Demonstrates expert knowledge in diabetes care and demonstrates knowledge of chronic disease self-management.
- Demonstrates excellence in interpersonal, communication, coaching, mentoring and team building skills that facilitate positive interactions among all members of the health care team.
- Possesses analytical, problem solving, critical thinking skills and demonstrates superior decision-making and organizational skills.
- Demonstrates a strong patient and customer focused philosophy in all interactions.
- Demonstrates commitment to professional development.
- Demonstrates ability to provide leadership, support and critical analysis in stressful, complex clinical and emotional situations.
- Demonstrates ability to implement evidence-based best practice into Policy and Procedure development and clinical practice.
- Demonstrates excellence in written and oral communication.
- Communicates effectively with the interdisciplinary team and external areas using timely, appropriate written, verbal and non-verbal means.
- Utilizes effective listening skills to maximize understanding of issues.
- Experience and demonstrated ability to work in a highly computerized environment.
- Proven attendance record. Ability to perform the essential duties of the job.
