Registered Practical Nurse - Resource Utilization Assessor - MRHH C3 Purple Mental Health

Permanent Full Time

Reporting to the Patient Care Manager - MRHH Inpatient Mental Health, and Patient Care Manager - Outpatient Mental Health, RPN - Resource Utilization Assessor role is viewed as a supportive role to the Resource Utilization Coordinator, enabling and role modeling teamwork and communication, autonomous clinical practice, data collection standards and a healthy workplace. This role will also include supporting the Outpatient Psychogeriatric Clinic.

Contribute to safe, quality care by

  • Ensures all outstanding RAI assessments are complete, accurate and submitted to CIHI and IDS each fiscal quarter.
  • Coordination of timely and correct data input to promote Quality Improvement initiatives.
  • Collects statistics as required and submits quarterly statistical reports to the Resource Utilization Coordinator, Program Managers, Program Director, Decision Support and Finance.
  • Critically analyzes statistical data in relation to clinical outcomes and quality improvement initiatives.
  • Liaises with Resource Utilization Coordinator, Patient Care Managers, Patient Care Coordinators, and Finance to ensure data input and retrieval occur as required.
  • Completes therapeutic psychogeriatric mental health assessments.
  • Screen / triage psychogeriatric referrals to the outpatient program to determine how to serve patient best.
  • Facilitate Outpatient psychogeriatric groups.

What must you have?

  • Current Registration with the College of Nurses of Ontario as a Registered Practical Nurse.
  • Practical Nurse Diploma from an approved nursing program.
  • Minimum 1 year of inpatient mental health experience
  • Proficiency with Microsoft Excel, Word and PowerPoint
  • Proficiency with EPIC EMR or similar EMR

  • Facilitates the effective delivery of competent, compassionate patient centered care by monitoring and evaluating patient care processes and outcomes.
  • Graduate of a recognized RPN program.
  • Experience in information technology and its application in a clinical environment.
  • Self-directed individual with strong analytical and problem-solving skills.
  • Knowledge and experience in OMHRS reporting, submission, error correction guidelines.
  • Experience in clinical outcomes in relation to quality improvement initiatives.
  • Ability to communicate and collaborate with all members of the inter-professional team.
  • Ability to support the clinical team in a professional manner.
  • Excellent organizational and prioritization skills.
  • Ability to problem solve clinical issues with the support of the Patient Care Manager.
  • Excellent critical thinking, decision making and conflict resolution skills.
  • Strong patient focused philosophy in all interactions.
  • Solid computer skills; experience with relevant healthcare applications.
  • Communicates in an effective, professional manner.
  • Proven attendance record.
  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
  • Ability to perform the essential duties of the job.

  • Member of the Registered Practical Nurses’ Association of Ontario (RPNAO) preferred.
