An opportunity is available for an Advanced Practice Provider (APP) to work within our innovative Holland Bone and Joint Program. 

 

The APP is an experienced orthopaedic health professional who delivers expert-level clinical practice within an interprofessional model of care. In collaboration with orthopaedic surgeons and other members of the team, the APP facilitates timely access to quality care and provides referral to appropriate treatment and specialist consultations for patients referred to Sunnybrook.  The APP is a champion for best practice, and innovative care models and contributes to quality improvement initiatives to achieve safe, effective, efficient care. The initial focus is on Hip and Knee Arthritis with potential to expand to additional MSK patient populations.

Summary of Duties

  • Working collaboratively with the orthopaedic surgeons and other members of the interdisciplinary care team, the APP:
  • Performs comprehensive physical assessment and triage of patients referred with hip and knee arthritis to determine need for surgical consultation and optimize conservative management
  • Performs post arthroplasty reviews on behalf of the surgeons, consults with orthopaedic surgeons as indicated, and makes recommendations for ongoing care
  • Serves as a clinical consultant for the interprofessional team and acts as resource for referral sources and community service providers.
  • Orders appropriate diagnostic testing to support assessment, diagnosis and decision-making
  • Provides education and recommendations to patients, families and their primary care providers to support the management of their condition and optimize function in keeping with best practice guidelines
  • Applies current knowledge of a wide range of treatment options to formulate/deliver an individualized treatment and management plan across the care continuum  
  • Consults with orthopaedic surgeons and others on the care team, to establish appropriate care for patients with complex presentations and needs
  • Directs care onward to surgeons/other medical specialists when indicated 
  • Teaches medical residents and others, MSK assessment and management skills 
  • Contributes to the development, evaluation and improvement of team-based care and practice innovations
  • Participates in research to address problems and issues arising from practice
  • Acts as an ambassador for the program

 

Qualifications/Skills

  • Current registration with respective professional College (e.g. College of Physiotherapists of Ontario, College of Occupational Therapists of Ontario)
  • Master’s Degree (beyond entry level) or post professional formal education with competency evaluation
  • Minimum 5 years Orthopaedic physical assessment/treatment experience, minimum one year of experience in an advanced practice role
  • Expert knowledge, complex decision-making skills and clinical competencies to form a foundation for expanded practice
  • Evidence of effective program development, outcome evaluation and clinical research
  • Collaborative approach to teaching/learning in an interprofessional environment
  • Demonstrated professional leadership, innovation, team building
  • Demonstrated ability to deal effectively with change
  • Excellent interpersonal, oral and written communications skills
