Summary of Duties
We have a temporary full-time vacancy (8 months) for a Pharmacist in the Acute Care Pharmacy with patient care responsibilities to the C2 Oncology service.  Start Date:  as soon as possible 

The pharmacy duties can be broadly divided into three areas:
1.    Drug distribution
2.    Direct patient care.
3.    Educational responsibilities 

Responsibilities include physician order review, clarification and entry;  drug dispensing and or supervision of such tasks by pharmacy technicians; chemotherapy pharmacist responsibilities including chemotherapy drug dispensing and/or supervision of such tasks by pharmacy technicians; answering of drug-related questions; provision of pharmaceutical care; medication reconciliation; participation in multidisciplinary rounds; patient counselling; activities associated with medication funding; participation in the education of students and multidisciplinary staff; safety report initiation and follow-up; other pharmacy duties including participation in on-call activities and hospital and departmental committees. This pharmacist will be responsible for working closely with Odette Cancer Centre Pharmacists.  

 

Responsibilities to this unit will include:  best possible medication history (BPMH) and medication reconciliation, medication counselling for all new patients starting parenteral and oral chemotherapy,  discharge counselling and coordination of medication access, drug reimbursement coordination, chemotherapy protocol review and development, support for ongoing computer systems associated with chemotherapy ordering.    

Qualifications/Skills

•    Recent prior work experience in the Acute Care pharmacy, Sunnybrook Health Sciences Centre or Acute Care hospital pharmacy environment preferred
•    Current licensure by the Ontario College of Pharmacists
•    Prior work experience in an in-patient hospital Oncology unit and hospital pharmacy residency training preferred
•    Able to work well in a fast-paced, sometimes stressful work environment
•    Self-motivated and able to take initiative
•    Must work effectively with minimal direction or within a team setting
•    Demonstrated ability to work well in a multidisciplinary team environment
•    Excellent interpersonal and communication skills
•    Excellent attention to details
•    Strong planning and organizational ability; strong time management skills
•    Strong customer service skills
