Summary of Duties
Sunnybrook Health Sciences Centre has a vacancy for the position of Risk Manager within the Department of Quality, Patient Safety and Enterprise Risk. Reporting to the Manager, Enterprise Risk Management, the incumbent will work collaboratively with the risk management and quality and patient safety teams, senior leadership, directors, managers, front-line staff and various internal and external stakeholders across all of Sunnybrook’s campuses to address risk management issues and implement organization-wide policies and other initiatives to promote patient safety and mitigate risk.
Risk Management Duties:
Operational Risk Management and Patient Safety:
- Responds daily to organization-wide risk requests to provide expert consultation and advice on a wide variety of patient care, policy and administrative matters with internal constituents including senior leaders, managers, physicians, clinical staff, corporate support departments and patient safety.
- Reviews/audits/edits Safety Event Reports and ensures accuracy, due diligence in the investigation and follow up by/with stakeholders as applicable. Monitors Safety Event Reports for trends and brings same forward to applicable stakeholders.
- Provides risk consultation to develop and revise policies, procedures and select Emergency Response Codes to avoid and/or control risk, in collaboration with the Emergency Preparedness leads.
- Provides onsite leadership at select emergency codes and unplanned infrastructure disruptions to ensure effective communication, response, and return to business operations.
- Executes an effective response to external sources of risk identification e.g., Coroner’s requests, jury recommendations, regulatory college complaints, product hazard and recall notifications and look backs, etc.
- Participates in and/or leads key risk and patient safety related committees i.e. System Review/QCIPA committee, Police-Hospital Liaison Committee, and participates in select corporate level committees as a Risk Management representative.
Quality of Care Reviews:
- Leads the risk response to serious safety events and critical patient incidents including supporting clinical and non-clinical teams, liaising with patients/families, conducting investigations, liaising with hospital leadership, and leading QCIPA and non-QCIPA system reviews (quality of care reviews) through to the implementation of recommendations.
- Supports the development of reports and presentations for system review recommendations to the QCIPA Quality of Care / System Review Committee
- Shares system level recommendations for improvement with operational and senior leadership for feedback and endorsement
- Provides support and guidance to clinical teams to ensure appropriate steps taken regarding disclosure and sharing of recommendations with patients and families.
Claims Management:
- Leads the identification and management of potential and actual professional liability, malpractice and personal injury claims.
- Leads investigations of claims with hospital adjuster and legal counsel; represents hospital at Examinations for Discovery; supports and guides staff defendants, their managers and others through the legal process.
IRM:
- Supports the organization’s Integrated Risk Management (IRM) program through engagement with leaders regarding new and existing enterprise risks, mitigation strategies and implementation of controls.
- Utilizes the HIROC IRM Risk Register application to ensure accurate documentation of risk assessment, progress and action planning related to organizational risks.
Risk Mitigation:
- Ensures the annual completion by all stakeholders of the Risk Assessment Checklists (RAC) required by hospital’s Insurer, HIROC, and tracking follow-up action.
- Assesses, identifies and mitigates risk associated with select planned high-risk shutdowns of services or utilities and select facilities renewal projects, by providing high-level guidance on effective contingency planning with stakeholders.
- Identifies and mitigates risk at every opportunity with a focus on improving patient, staff and public safety.
- Ensures appropriate actions are taken to address identified deficiencies/areas for improvement.
- Partners with internal and external stakeholders to maximize opportunities for collaboration and integration of best practice and ensure the hospital’s risk management processes are based on best practices and meet legal and regulatory requirements
Qualifications/Skills:
- Health Care and/or Health Sciences related undergraduate degree required (clinical background with minimum of 5 years of acute care experience preferred, Master’s degree an asset). Current registration with a recognized health profession college or regulatory body if applicable
- Minimum 4 years’ experience in healthcare risk management and claims management strongly preferred
- Formal training or certification in Patient Safety and/or Quality Improvement preferred
- Healthcare Risk Management Diploma or Certification from an applicable program completed or in progress (e.g. Accreditation Canada, OHA, HIROC, Osgoode School of Law, etc.) is considered an asset.
- Previous experience with online RL incident reporting programs, electronic patient charts, HIROC Risk Register an asset
- Knowledge of the Canadian Incident Analysis Framework
- Excellent critical thinking, attention to detail, analytic and creative problem-solving skills
- Demonstrated ability to prioritize various demands in a dynamic and fast paced work environment and to manage a number of projects simultaneously
- Self-motivated with demonstrated ability to communicate well with all levels of staff, physicians, volunteers, families and community partners
- Demonstrated facilitation, conflict resolution and negotiation skills to lead groups to consensus and resolve conflict
- Demonstrated interpersonal, diplomacy, and communication skills (written, verbal and presentation)
- Proven Experience in preparation of educational materials, teaching and/or formal presentations to large groups at all levels of the organization.
