Company Biography:

Scarborough Ontario Health Team (SOHT) is one of the largest OHTs in the province and based in one of Canada’s most vibrant and diverse communities. We are a network of over thirty partner organizations with a rich history of collaboration providing health and community services across the continuum of care. Our OHT’s attributed population of almost 900,000 goes beyond the residents of Scarborough to include residents of neighboring communities that have made Scarborough their health care destination of choice.


SOHT partners are working together with the hundreds of physicians in the Scarborough Family Physicians Network (SFPN) and our active Client and Family Advisory Committee (CFAC) to co-design innovative, integrated solutions to providing care. SOHT is governed by a 24-member “Collaboration Council” that includes individuals from SFPN, CFAC and partners from across all health sectors. The Collaboration Council is co-chaired by a representative of the lead agency (Scarborough Health Network) and a primary care physician representative of SFPN.


We are strongly committed to reducing health and social inequities to achieve improved population health. We provide integrated services for frail seniors living with mental health and/or addictions issues and multiple chronic conditions, and provide support for caregivers. While we face some hard challenges when it comes to social determinants of health, access to proper care and health literacy, we also have the passion, drive and determination to meet these challenges head on. SOHT is a strong advocate of the Scarborough Family Physicians Network (SFPN), and committed to the ongoing relationship with SPFN by providing staffing and administrative support to this incorporated organization. The Scarborough Family Physicians Network is the unified voice of primary care physicians in Scarborough. They are actively working to improve Scarborough’s primary care physician and patient experience. This will ensure that patients have timely access to specialist and allied health care with services tailored to the Scarborough population.

As the lead agency for the SOHT, Scarborough Health Network (SHN) is proud to invite you to apply to the Coordinator, Scarborough Ontario Health Team position. This position is a full-time term contract until September 2025, subject to provincial funding and directives.

Position Overview:

  • The SOHT Coordinator is responsible for supporting the SOHT team in the execution, implementation and tracking of various initiatives, operational tasks and implementations across the SOHT portfolios. Responsibilities:

  • Provides input and actively participates in idea generation and decision-making processes

  • Participates in stakeholder engagement, outreach, and facilitates meetings

  • Works collaboratively with team members and partner organizations on initiatives

  • Participates in research, data collection, and provides data analysis

  • Creates and updates project implementation dashboards for leadership and governance reviews

  • Liaises with leadership and team members to ensure completion of project deliverables

  • Works in collaboration with stakeholders to develop strategic agendas, goals, and meeting materials

  • Ensures projects are aligned with OHT strategic priorities

  • Effectively captures key decisions and follows up with stakeholders to ensure completion of action items

  • Extracts data from Ministry of Health, Ontario Health, or OHT partners and presents it visually for SOHT leadership or external stakeholders

  • Manages meeting invites and distribution lists

  • Responds to email inquiries from agencies and members of the public in an appropriate and timely manner

  • Maintains and organizes SharePoint (i.e., uploads and ensures appropriate storage of records and access to files for relevant team members)

  • Effectively communicates key documents, information, and updates to stakeholders

  • Updates financial tracker and manages invoice processing

  • Prepares agendas, minutes, reports, and presentations for committees and governance meetings

  • Creates spreadsheets for data collection and financial tracking

  • Creates and disseminates monthly newsletters to partner organizations

  • Drafts proposals, briefs, reports and submissions for regional or provincial reports and/or funding proposals

  • Updates websites and social media channels

  • Supports/leads data collection, analysis, and provides recommendations

Education, Qualification and Experience Requirements:

  • Post-Secondary Degree in Communications, Business Administration, Office Administration or equivalent

  • A minimum of 2 years work experience in project management, specifically project coordination and administrative tasks in a health care setting

  • Health care experience an asset

  • Experience in drafting proposals, writing briefs, and budget tracking and monitoring

  • Ability to develop reports and presentations for various stakeholders

  • Strong working knowledge of computer applications, notably Microsoft Office

  • Exceptional written and verbal communication skills, as well as attention to detail

  • Critical thinking and strong ability to work independently and proactively anticipate needs of work that needs to be done

  • Exceptional organizational skills with the ability provide administrative support to multiple projects and stakeholders while constantly adjusting priorities

  • Demonstrated excellence in interpersonal, written and verbal communication skills including, ability to demonstrate tact and diplomacy with diverse groups of stakeholders, including patients, front-line healthcare providers, community agencies, and healthcare administrators

  • Attention to detail and high level of accuracy

  • Maintain a high degree of confidentiality
