The Community Health Worker (CHW) will collaborate with clinicians and community-led organizations on the development of a comprehensive plan to help address individual client’s social and other needs.

The Community Health Worker will work at UHN and in community settings to provide direct 1:1 support to marginalized clients through the collaborative coordination of health and social services. The Community Health Worker will support the provision of information, resources, and advocacy, and will participate in case discussions and debriefs. The Community Health Worker will provide support to the UHN SUIT team and other areas of UHN to build, grow, strengthen, and sustain relationships with community partners; and provide supports to staff around the needs of marginalized clients.

The SUIT Team CHW is a member of the broader Addiction Services Program and applies the principles of harm reduction to their work with patients. This position will also provide occasional support to the Rapid Access Addiction Medicine (RAAM) service.
