The Physiotherapist (PT) is an integral member of the Specialized Dementia Unit. Through the provision of patient and non-patient care activities, the PT enhances the quality of life of patients by assessing, planning and implementing individually designed programs of physical treatment to maintain physical functioning in patients. Principal responsibilities include: providing assessment, intervention, consultation and education services for patients; collaborating with inter-professional team in providing patient focused care; communicating effectively with patients, families, colleagues and external stakeholders; developing and implementing creative comprehensive patient-centred care plans based on evidence and Best Practice standards; consulting with internal and external stakeholders regularly to facilitate smooth and timely patient transitions; participating actively in the planning, implementation and evaluation of the program; participating in professional and organizational initiatives supporting best practice, quality management and research; providing education to students, as appropriate.
