Vancouver Coastal Health is seeking a dedicated and compassionate provider (GP/NP) to join an innovative outreach role which will support clients in supported housing. This role is critical in providing episodic care to diverse populations across our community, focusing on urgent health needs with a strong emphasis on mental health, substance use, and systems coordination.

As a General Practitioner with Vancouver Coastal Health, you will:

In this role you will act as the MRP for patients in the following housing models:

Victory House (hybrid):

  • Longitudinal primary care for 45 total clients (34 of them are attached to the Strathcona Mental Health & Substance Use team with chronic, severe mental health issues)
  • To round in person 1 week a month with virtual support otherwise from 9-5 Monday-Friday
  • Time: 4 hours a week

Complex Care Housing (hybrid):

  • Episodic/bridging primary care for vulnerable clients in complex care housing sites across Vancouver
  • These sites have nurses embedded who would request consultation for episodic care issues as well any primary care needs that come up
  • This work could be done virtually or in person with an outreach visit
  • Time: 8 hours per week

Cambie Gardens Complex Support Housing (in-person):

  • Longitudinal primary care for 44 complex care patients
  • To round in person 1 week a month
  • Time: 4 hours per week

Mobile De-Escalation Team (remote):

  • A same-day crisis response team for individuals in the inner city who are experiencing a mental health or substance use crisis
  • Provide on-call, virtual, episodic care to patients, who will be initially assessed by our nurses in-person on Saturdays or Sundays from 0800-1900
  • Time: 10 hours per week
Qualifications

Education, Licensing, & Experience:

  • Current College of Physicians of BC (CPSBC)- Provisional
  • Canadian College of Family Practitioners (CCFP) - Eligible
  • Canadian Medical Protective Association (CMPA)- Mandatory

Knowledge & Abilities:

  • Provide episodic care to patients who will be initially assessed by their respective housing site’s nurse
  • This could be done virtually or in-person
  • Liaise with nursing staff create an episodic care plan for a patient and determine which services they will be bridged/connected to
  • Coordinating with the other physicians/NPs and/or other VCH outreach teams as needed to ensure continuity of care

This episodic care could require:

  • Skin and soft tissue infection (SSTI) management
  • Bridging scripts for opiate agonist management
  • Antipsychotic titration for mental health decompensation
  • Virtual certification with a Form 4 (on rare occasions)
  • Care coordination, as well as liaison with specialists

Requirements:

  • MD degree with a current license to practice in British Columbia.
  • Experience in primary care, emergency medicine, psychiatry, or related fields.
  • Proficiency in virtual care platforms and electronic medical records.
  • Excellent communication and interpersonal skills.
  • Commitment to patient-centered care and harm reduction principles
  • You will need to use Profile EMR, PARIS as well as Cerner for this role, and will be compensated for any training required.
