Dietitian/Nutritionist 1 (DT01) and Diabetes Case Manager

JOB SUMMARY:

Reporting to the Chronic Disease Manager, the incumbent will collaborate with physicians in their primary health care practice, with Vitalité Health Network’s primary health care teams, and with the community by providing specialized clinical expertise in diabetes management and related complications for patients whose diabetes is uncontrolled and who pose unique and complex challenges. The incumbent will act as a resource person for physicians, primary health care providers and other community groups with respect to best practices in diabetes management. The incumbent will, in addition, make a major contribution to promoting this non-traditional role and its inclusion in physicians’ offices. The incumbent will also act as a leader in the development, coordination, delivery and evaluation of this new service delivery model. 

REQUIREMENTS:

  • Member in good standing of the New Brunswick Association of Dietitians (NBAD);
  • Bachelor of Science in Nutrition or equivalent from a university recognized by the Dietitians of Canada;
  • BLS (basic life support) certificate and maintenance of certification;
  • Minimum of five years of clinical experience with a clientele suffering from complex, chronic conditions, including two years with diabetic patients (preferably within the past seven years);
  • Certified diabetes educator;
  • Certification from the NB Insulin Adjustment Certification Program;
  • Ability to read and analyze insulin pump reports and continuous glucose sensors would be an asset;
  • Demonstrated clinical knowledge of diabetes management, including associated complications and impact on the emotional, psychological and social health of patients and their families;
  • Demonstrated ability to develop care plans around objectives;
  • Demonstrated knowledge in self-management and ability to use motivational interview principles;
  • Strong interest in furthering own knowledge;
  • Demonstrated leadership skills, including strong interpersonal and oral and written communication skills;
  • Ability to recruit professionals to work collaboratively;
  • Demonstrated ability to integrate recent research work into clinical practice;
  • Knowledge of the health care system and ability to help patients and their families navigate the system;
  • Excellent group facilitation and presentation skills;
  • Excellent time management skills, strong organizational skills, effective work methods and ability to prioritize and manage several activities at the same time;
  • Skills using computer tools and Microsoft Office programs (Word, Excel, PowerPoint);
  • Valid driver’s licence and good driving history combined with availability to travel within the zone;
  • Ability to work independently and as part of a team;
  • Physical ability to perform assigned work;
  • Good work history (performance and attendance);
  • Adherence to professional ethics principles, the Network’s management philosophy and organizational values;
  • Adherence to Vitalité Health Network’s confidentiality rules.
