PASS - Special Education Transition Navigator (1.0 Year Contract)

The Waterloo Catholic District School Board serves over 38,000 learners and provides services to students in 50 schools (Kindergarten to Grade 12 and adult learners) across the Waterloo Region.

General Description of Duties:

Reporting to the Principal of Student Services, the Special Education Transition Navigator will support transition practices for students with special education needs and/or disabilities into, during and out of school.

Liaise and communicate with community agencies and organizations including committee membership and establishment of community partnerships.

Assist families in navigating the transition from elementary to secondary school and support families in connecting with community agencies.

Share best practices/resources/tools that will help support successful transitions and supports for students with special education needs and/or disabilities.

M inimum Requirements:

A post-secondary degree in a social service worker and/or related field, or the equivalent combination of formal training, specialized coursework and related experience

Three years of related experience

The ability to clearly communicate in both verbal and written form and possess excellent interpersonal skills

Knowledge and experience working with relevant community agencies and organizations

Working understanding of Ontario Disability Support Program and Developmental Services Ontario and other funding opportunities

Knowledge and working understanding of Ontario Autism Program

Demonstrated project management skills and the ability to work with minimal supervision

Proficiency with technology and Microsoft Office Suite

Proven organizational and time-management skills

Knowledge of Ministry of Education initiatives and Ministry of Children, Community and Social Services initiatives

Valid G driver’s license and have access to a reliable vehicle

Salary Range: Level 4 (42 weeks), $53,559.00- $63,011.00 per year in accordance with the PASS Terms of Employment

Hours of Work: Full-time, 35 hours per week / 42 weeks per year - (1) year appointment, ending June 27, 2025

Location(s): Catholic Education Centre, 35 Weber Street West, Unit A, Kitchener, Ontario (currently staff are working between the Catholic Education Centre, schools and remotely)
