1.0 EFT (38.75 hours per week)

Salary Range $40.536 TO $52.900 depending on qualifications

Job Summary

Reporting to the Director of Primary Care the job is full-time, permanent, 77.5 hours per two-week period (including holiday pay hours). The regular workdays for this position will fall between 8:00 a.m. and 6:00 p.m. Mondays through Fridays. Occasional evenings and weekends may be required.

The position is classified as MAHCP. A comprehensive benefit package is provided as per employee eligibility via HEB Manitoba.

Key Responsibilities

The Social Worker is a member of the Primary Care Program of Nine Circles and an integral member of the clients’ Care Team.

The goals of the social work team are to:

  • Provide wrap-around care with psychosocial supports
  • Offer and link to culturally relevant supports
  • Support People Living with HIV (PLHIV) to access and adhere to HIV treatment medication

The Social Worker assesses clients’ needs, situations, strengths, and support networks to determine their goals. They help clients adjust to changes and challenges in their lives, respond to crisis situations, refer to and advocate for community resources. The social worker follows up with clients to ensure that their situations have improved and develops and evaluates programs and services to ensure that basic client needs are met.

Using a client centred, strength based, and harm reduction approach, the social worker supports psychosocial support in clinic and outreach settings including community settings, shelters, encampments and client’s homes.

Qualifications

Education

  • Undergraduate degree in Social Work
  • Active registration with the Manitoba College of Social Workers

Knowledge, Skills, and Experience

  • Three years of case management or relevant experience with people experiencing diverse barriers and complex challenges to good health outcomes
  • Commitment to and knowledge of community-based health care and the social determinants of health
  • Demonstrates ability to provide culturally safe and appropriate care; demonstrated knowledge/understanding of impact of colonialism on Indigenous patients/clients.
  • Demonstrated knowledge of sexual health, sexually transmitted and blood-borne infections, mental health and substance use issues, best practices and evidence-based practice.
  • Demonstrated knowledge of patients living with substance use disorders, mental illness and other chronic and complex conditions (e.g. HIV, Hepatitis C).
  • Demonstrated ability to undertake comprehensive psychosocial assessments that are informed by theoretical and conceptual frameworks, utilizing standardized tools and developing care plans in collaboration with clients/patients.
  • Ability to identify and carry out appropriate therapeutic interventions with knowledge/understanding of past and current trauma, substance use and substance use disorder, violence prevention and transgender care.
  • Ability to assess, plan, organize, implement, evaluate and problem-solve.
  • Ability to establish rapport with clients with multiple barriers.
  • Ability to communicate effectively with individuals, and other professionals.
  • Ability to work effectively with others independently and as part of an interprofessional team and understanding the frameworks of other disciplines.
  • Demonstrated ability to organize and prioritize workload in a complex environment.
  • Demonstrated comprehensive knowledge of relevant Acts such as Mental Health Act, Public Guardian and Trustee of Manitoba, Health Care (Consent) and the Manitoba Care Facility Act, Personal Health Information Act (PHIA), pertinent regulations, policies and procedures, and other statutory duties.
  • Ability to educate and provide consultation to patients/clients and healthcare professionals.
  • Ability to develop, coordinate and deliver presentations.
  • Ability to make effective referrals, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other community partners.
  • Comprehensive knowledge of client/patient-centered care, psychiatric diagnosis and professional practice guidelines for the treatment of various disorders.
  • Demonstrated knowledge of psychiatric illnesses, interventions and impact on social functioning.
  • Demonstrated therapeutic skills and the ability to apply theory and practice in the areas of case management counselling, individual therapy to patients/clients with a variety of mental health and substance use issues, and HIV/AIDS, HCV.
  • Proficiency in the use of personal computers, including related word processing and database management software applications.
  • Physical ability to carry out the duties of the position.
