As a case manager the Licensed Practical Nurse (LPN) is accountable for assigned caseloads with the primary responsibility to assess client needs, determine service needs, negotiate service options, make service recommendations and referrals, monitor service delivery including deficiencies in service delivery, care planning, manage reassessment/waitlists and discharge processes, and coordinate care transitions across care settings. The LPN Case Manager demonstrates knowledge and accountability for the following three client groups: Short Term Acute (STAC), Acute and Wellness who present with single unmet health needs. When managing client care, LPN case managers demonstrate the eleven principles of case management (AHS, 2010). LPN Case Managers are responsible and accountable to: Complete the requisite education including but not limited to Case Management and Care Planning Work with their Care Manager and mentor to guide their learning and development of skills required in the case manager role. Reflect and identify practice areas for improvement. Demonstrate knowledge and ability to communicate, consult and collaborate with the interprofessional team. Utilize critical thinking and critical inquiry throughout the nursing process identifying and seeking support when the needs of the client have changed unexpectedly, and the care required exceeds the individual’s competency.

As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

The LPN is responsible to assess, plan, implement and evaluate the needs of Home Care clients and their caregivers/families. There is also an expectation to be a mobile worker. This role requires travel to care for clients in their homes or in the community. You will be responsible to carry and move equipment, use, and operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing, and pulling) as a regular part of the job. Learning resources and training are provided to safely perform work. The Case manager LPN will work within their scope of practice when conducting assessments, determining service needs, negotiating service options, making service recommendations and referrals and as a result continues to align with the current Standard provincial Job Description for the LPN role. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) from the Heart and Stroke Foundation within the past year. Minimum of two years current nursing experience with one year in a community-based setting.

Completion of a post-basic Medication Administration Course for LPN’s. Recent work experience in a community-based setting. 3 years Acute Care experience preferred. 2 years Home Care experience preferred. Experience in case management practice and completion of McMaster AHS Case Management Education preferred.
