This position is for Brooks Continuing Care. This area is comprised of 24 long term care beds, 9 Subacute and 5 Community Transition Beds with a palliative care focus. This position will rotate in all areas of care. As a member of the CoAct team, you will provide a wide variety of nursing services to patients and families, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and families. The LPN provides safe and effective nursing care according to hospital policies and nursing care standards. Working within the full scope of practice, the LPN observes, monitors, reports, and documents changes in the patients physical, mental, and emotional state and assists the patient with activities of daily living. The LPN is responsible to collaborate as a member of the interdisciplinary health care team, delivering consistent quality care to meet the needs of the patient. Brooks is one of fastest growing communities in Alberta with a multi-cultural flavor unique to other municipalities. Residents enjoy a wide range of indoor and outdoor recreational activities including camping, boating, fishing, sailing, kayaking, water skiing (water sports) ice fishing and skating in winter, just to name a few. This position qualifies for the Canada Student Loan Forgiveness Program, which reimburses $4,000 per year (up to a maximum of 5 years) for working in the rural Brooks setting. Attention LPNs! As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team utilizing nursing processes, critical thinking, problem solving and decision making skills. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Completion of an accredited practical nurse education program. Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Minimum of one (1) year experience in adult continuing care nursing experience within the last 2 years.

Understanding of CoAct and collaborative care in the continuing care setting. Nonviolent Crisis Intervention.
