Reporting to the Care Manager of Seniors Central and COPD Home care in the Calgary zone, the Nurse Practitioner (NP) functions as an integral member of the Seniors Central, COPD and In Home Rehab team which uses a blended interdisciplinary/transdisciplinary model of care, as well as the broader IHC program. Depending upon need, a client may see any mix of staff and physicians in Home Care including: NP, Registered Nurses, Occupational Therapists, Physiotherapists, Pharmacists, and a Therapy Assistant.

As a Nurse Practitioner (NP), you will provide a full range of comprehensive health services to individuals across their lifespan. You will be responsible for integrating clinical skills associated with nursing and medicine to assess, diagnose, order, and interpret diagnostic tests, prescribe treatment, and perform procedures to manage and meet the health needs of patients. Additionally, you will utilize advanced practice nursing competencies related to Clinical Practice, Leadership and Optimizing Health Systems, Education, and Research to increase care quality, promote appropriate use of health services, and inform and influence healthcare systems. Successful completion of an approved Nurse Practitioner program, with a minimum of a Master's degree. Active registration with the College of Registered Nurses of Alberta (CRNA) on the Nurse Practitioner Registrar, including Provisional Graduate Nurse Practitioner licensure. Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

This is an ON-SITE position. Recent experience working as an NP with the older adult population. Demonstrated excellent written and verbal skills. Demonstrated continuing education in the area of older adults. Proven initiative, autonomy, conflict resolution, critical thinking skills. Demonstrated compassionate, empathic and caring attitude. Demonstrated self-directedness in organizing work priorities. Demonstrates AHS competencies when working with patients/families, staff, and students. Demonstrated flexibility in meeting the unexpected spontaneous needs of a clinical area. Demonstrated ability to function effectively and collaboratively within a client and family centered team of health care professionals and support staff. Proven experience with teaching, supervision, coaching and mentorship. Ability to collaborate with stakeholders within and outside of AHS. Intermediate computer skills including the use of Microsoft office and other clinical information systems. Physically able to perform all required tasks e.g. carrying charts and equipment. This role requires the employee to sit for long periods of time. Able to lift up to 30 lbs. This position requires travel within the city of Calgary.

CNA certification in Gerontology. Minimum 3 years of experience with older adults in the community within the last 5 years. Experience completing autonomous home visits. Experience with ambulatory or community/Home Care. Experience in nursing leadership roles. Experience in research and best practice implementation.
