Social Worker II

This Social Worker II position provides social work services as part of inter-disciplinary team to clients primarily on the acute care transition unit located in Continuing Care at the Cold Lake Healthcare Centre as part of a multi-disciplinary team. Key components of this position are social work support for AHS system navigation, placement/transitions in care, and financial supports.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Certification in Basic Life Support - Healthcare Provider from the Canadian Heart and Stroke Foundation.

One year experience working a medical social worker in an acute care setting. Enhanced knowledge of community-based supports and services. Experience and knowledge related to AHS system navigation, placement and transitions in care and financial supports.
