Alberta Health Services Central Zone is looking for qualified candidates to fill the position for a Social Worker II, within the Transition Services and Seniors Health Portfolio. The successful candidate will work both independently and with multi-disciplinary teams providing family and patient centered care within the acute care and community portfolio. The Social Worker provides psychosocial services using a collaborative approach to support clients, families, and health professionals across the continuum of care, including Acute Care, Home Care, Supportive Living and Long-Term Care. This support may include, but is not limited to, supportive counselling, resource-based interventions completing psychosocial assessments, comprehensive risk assessments, cognitive screening, supporting transitions and discharge planning. The Social Worker will also provide education to clients and their families around substitute decision-making, resources and supports within and external to Alberta Health Services.

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Completion of provisional registration requirements preferred. Minimum 2 years of experience. This position requires a high degree of independence, critical thinking, and problem-solving skills, an ability to prioritize workload. Knowledge and ability to use computer applications such as Microsoft Office, Outlook and Teams is highly recommended. Previous experience working in the health care system is an asset.

Excellent communication skills, written and verbal. Experience working within an interdisciplinary team is preferred. Knowledge of relevant legislative Acts and experience with guardianship, trusteeship, personal directives and capacity assessment processes is an asset. In-depth knowledge and experience in working with Income Support programs at a community/provincial/federal level (AISH, Alberta Works, CPP, OAS, ASB) is preferred.
