The successful applicant will work in compliance with all relevant and applicable legislation and Hospital Policies and Procedures, will practice the Bluewater Health Standards of Performance Excellence and will fulfill their role within the Attendance Support Program parameters. The successful applicant will also possess effective communication and interpersonal skills, will have the ability to work independently and as part of a team, and will have strong attention to detail and accuracy.

Qualifications:

• Diploma from an accredited College in a Social Service Worker program or equivalent required
• Certification in Non-Violent Crisis Prevention and Intervention (NVCPI) preferred (required within 3 months of start)
• Valid Ontario unrestricted Class ‘G’ Driver’s license required

Skill, Ability and Experience:

• Strong working knowledge and understanding of Community Treatment Orders, the Ontario Mental Health Act, the Substitute Decisions Act, and the Health Care Consent Act preferred
• 1- 3 years recent experience working in a community-based mental health setting preferred
• Demonstrated ability to work collaboratively with both hospital and community based supports (ACT, CMHA, etc.)
• Skilled in use of audio-visual equipment for educational presentations required
• Intermediate level proficiency in the use of computers (MS Word, PowerPoint, Outlook, Excel) required

Principal Duties:

• Assists in the coordination, development and organization of Community Treatment Orders for clients and families
• Provides intake and screening for new clients / referrals for Community Treatment Orders
• In collaboration with client and/or substitute decision makers, develops a Community Treatment Plan that will maintain the client in his/her home and community
• Coordinates ongoing support services for clients in conjunction with adherence to the Community Treatment Plan
• Schedules and coordinates ECT treatments for both inpatient and outpatient clients of Bluewater Health.
• Facilitates ongoing education for patients, families, community stakeholders and members of the Mental Health Program including education & information sessions across Lambton County re: Community Treatment Orders.
• Participates in Mental Health Inpatient Rounds and supports creative problem solving to identify patients who may be appropriate for CTO
• Acts as a liaison with the Bluewater Health Psychiatric Assessment Services, ACT Team, and CMHA
• Establishes and maintains strong linkages with physicians/psychiatrists, community partners, and program staff involved with clients
• Participates in education on development of new, evidence-based approaches for CTO coordination
• Other duties as assigned
