The Complex Care & Recovery Program requires a full-time, temporary (6 months) Social Worker 2 whose primary responsibilities will be treatment and discharge planning for an all-male general forensic unit. Working with clients under the Ontario Review Board and reporting to the manager, the successful candidate will be expected to work collaboratively in an interprofessional team setting. You will be responsible for providing psychosocial assessments, time limited individual and group treatment to patients and families from diverse backgrounds. Your sound knowledge of community and Centre-wide resources informs your discharge planning activities - particularly in addressing the clients’ housing, income and aftercare needs. You will be responsible for the participation in the formulation of treatment plans and the preparation of Ontario Review Board reports. Candidates must be capable of providing supervision of Masters of Social Work students, collaborating with faculties of social work on special projects related to the development of the social work field and also teaching other professionals. Workload data entry is a job requirement. You will support and contribute to a healthy workplace that embraces diversity, encourages teamwork and complies with all the applicable regulatory and legislative requirements. You will be working closely with the Forensic Outpatient Services team to ensure adequate resources are in place for clients transitioning from the inpatient setting. This position is located at 1001 Queen Street West.

The successful candidate will possess a Master’s of Social Work degree combined with a preferred 3 years of post-graduate work experience and be registered and a member in good standing with the Ontario College of Social Workers and Social Service Workers. You will be eligible for an academic cross appointment. You are a mental health clinician with demonstrated experience in facilitating discharge planning, conducting assessments, and providing psychosocial support and psycho-education. Preference will be given to those who possess a strong knowledge of and ability to apply principles of risk management, CAMH follow up, community resources and crisis intervention. Candidates with sound knowledge and understanding of social work practice with a forensic system and its client population are preferred. Ability to work with Families, clients who are homeless/street-involved, and with those affected by substance use/abuse/addictions is an asset. Experience working with individuals from diverse ethno-cultural backgrounds is required. You will also possess excellent organization, communication and interpersonal skills and the ability to work effectively as a team member. Experience working with individuals from diverse ethno-cultural backgrounds is also required. Bilingualism (French/English) is an asset.
