Job Summary

Fraser Health invites you to join an innovative and unique primary health care team in one of Canada’s fastest growing and culturally diverse communities! We are currently recruiting full-time and part-time family physicians to provide comprehensive, integrated, and trauma-informed care at our Urgent and Primary Care Centre located in Port Moody, B.C.

 

Our Urgent and Primary Care Centre opened February 2021 and is located temporarily at Eagle Ridge Hospital. A new permanent site dedicated to delivering timely, person-centred, culturally safe, trauma informed longitudinal and urgent primary care in Port Moody is targeted to open this summer (2022).

 

Our model of care offers holistic team-based care by supporting family physicians and nurse practitioners with allied health professionals including nurses, clinical counselors, social workers, pharmacists and administrative staff. Together this team will extend continuous care that will be intentional and coordinated to all members of the community, including the vulnerable and medically complex.

Port Moody is located within the Tri-Cities, with extensive access to parks, nature, transit, vibrant local culture, and growing, diverse communities.
