Humber River Health, located in the new Wilson site, a state-of-the-art, fully digital facility, is seeking a dynamic, evidenced-based Patient Flow Manager. This experienced individual will coordinate and facilitate the seamless flow of internal and external patient admissions, transfers, and discharges that ensure “the right patient is placed in the right bed at the right time”, and maximize the utilization and allocation of patient beds and clinical resources within our 24/7/365 clinical operation.

Position Responsibilities:

  • Perform Patient Flow Manager responsibilities in the Emergency Department, Corporate Patient Flow, and provide patient quality of care support as a Clinical Expediter
  • Coordinate patient flow from internal and external patient admissions, transfers, and discharges to ensure all patients are admitted and referred to the most appropriate services for their clinical needs, and resolve patient flow issues as they arise.
  • Monitor, escalate and support timely patient care and quality through oversight of wall of analytics data related to patient deterioration, seniors care, sepsis, and other clinical risk issues
  • Coordinate and redeploy staff in the ED and across the hospital to achieve appropriate skill mix based on patient safety, departmental coverage needs, and associated risks.
  • Collaborate with Emergency Department and Inpatient Managers to identify process improvement strategies to improve the current system and reduce patient length of stay (LOS).
  • Define and prepare reports to inform management, physician chiefs, and administrative team of patient activity and bed utilization to manage surges in patient volumes.
  • Manage after-hours clinical services to act as night manager and primary crisis contact for all clinical activity areas in hospital (e.g. developing discharge plans, resolving family conflicts, emergency codes, incident management).
  • Identify and communicate clinical concerns or systems issues that occurred during evening or weekends to managers for action.
  • Facilitate and escalate in order to complete patient flow with a focus on clinical activities.
  • Expedite, prioritize, synchronize and mediate clinical issues, barriers.
  • Review and respond, in real time, to clinical indicators for patient care delays.
  • Track, prioritize and facilitate key discharge related workflow activities.
  • Track and escalate conditional discharge orders (MI, Lab, etc.) in collaboration with the MI Coordinator
  • Track and prioritize allied health and physician consults.
  • Review and respond to clinical indicators for patients falling off clinical pathways.
  • Define and prepare reports to inform management, physician chiefs, and administrative team of patient activity and bed utilization to manage surges in patient volumes
  • Review/Approve Observer requests requiring Patient Flow Manager’s approval after hours
  • Approve overtime, agency use after hours

 

Qualifications:

 

  • Current registration and membership in good standing with the College of Nurses of Ontario (CNO)
  • A Baccalaureate Degree in Nursing required
  • Membership with the Registered Nurses Association of Ontario (RNAO)
  • Master’s in Nursing, Health Science or related field is preferred. Candidates who are actively pursing their master’s will be considered.
  • Minimum of five years clinical experience and a minimum of three years leadership experience in an acute care setting.
  • Exceptional analytical and information-seeking skills that contribute to effective decision-making and strategic planning and execution of action plans that deliver results and motivate individuals for greater performance excellence
  • Ability to work with a diverse stakeholder group with demonstrated facilitating and coaching clinical areas on change initiatives
  • Self-directed, courageous, and highly motivated with excellent interpersonal and communication skills
  • Experience managing competing demands in a fast-paced, multi-site, multi-disciplinary health care environment
  • Proven risk management strategies
