Description
In collaboration with the interdisciplinary team, the incumbent will provide comprehensive psychiatric assessments and consultation to determine the appropriate treatment needs and disposition of, (1) psychiatric patients presenting in the Emergency Department, (2) patients referred for internal psychiatric consultation within Mackenzie Health, and (3) patients referred to the Adult Outpatient Mental Health Urgent Clinic.
Contribute to safe, quality care by:
- Manage facilitating linkages as appropriate to services and supports in the Hospital and the wider community.
- deliver patient/family focused care with an emphasis on assessment, education, prevention, maintenance, and restoration of health in a supportive and culturally sensitive manner.
What must you have?
- Graduate of a recognized diploma/degree program in nursing with a certificate in Mental Health/Psychiatry (certificate preferred) or Masters of Social Work degree from accredited University.
- Current Registration with the College of Nurses of Ontario or Member of Ontario College of Certified Social Workers (OCCSW).
- Minimum of 5 years recent in Mental Health, with clinical experience specific to Emergency Department.
What else do you bring?
- Demonstrated crisis intervention skills for both telephone and person-to-person situations.
- Working knowledge of the network of the inpatient services and community-based resources for children and adults with mental health needs.
- Complete intake assessment.
- Provide team leadership towards supporting current model of service and coordinates intake process with Patient Flow processes.
- Provide consultation and clinical support as required to colleagues and community agencies on an individual basis to facilitate resolutions to issues.
- Liaise with members of inter-professional team to ensure excellence in client care.
- Respond to all information requests through telephone, mail, and email, for prospective clients and their caregivers.
- Demonstrated leadership skills, sound judgment and diplomacy.
- Strong patient and customer focused philosophy in all interactions.
- Possess the ability to implement strategies that effectively facilitate change.
- Strong assessment skills and ability to coordinate services in a recovery oriented service.
- Comprehensive knowledge of community mental health networks.
- Experience and understanding of the needs of the mental health populations.
- Consistent demonstration of sensitivity and compassion to client and caregiver concerns and fears.
- Strong problem-solving and critical thinking skills with proven ability to work effectively with community partners.
- Computer literacy with developed skills in various software applications including MS Office, RAI, ESP.
- Demonstrated ability to manage high volume and varied work assignments with accuracy and flexibility and in a timely fashion.
- Knowledge and sensitivity to work in a culturally diverse environment.
- Self-motivated with demonstrated ability to function as part of a team and/or independently.
- Proven attendance record.
- Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
- Ability to perform the essential duties of the job.
- Effectively uses empathy in interactions with others.
