Description
The Nurse Practitioner (NP) as an autonomous health professional with advanced education provides essential health services grounded in professional, ethical and legal standards. The Geriatric Nurse Practitioner integrates in-depth knowledge of advanced nursing practice and theory, health management, health promotion, disease/injury prevention, and other relevant biomedical and psychosocial theories to provide comprehensive health services, with a focus on specialized geriatric assessments. The Nurse Practitioner works in collaboration with clients, the geriatricians and other health-care providers in the provision of high-quality patient-centered care. Works with diverse client populations and patient care functions, in a variety of contexts, across a multi-sited facility.
The RN (EC) has an expanded scope of practice that includes a range of diagnoses, prescribing a range of drugs and ordering a range of diagnostic tests. The RN (EC) role will encompass the full scope of advanced nursing practice blended with expanded functions including medical directives.
Contribute to safe, quality care by:
- Understanding scope of practice of the nurse practitioner along with responsibilities and accountabilities.
- Consulting with and/or referring clients to other health-care providers at any point in the care continuum when the client’s condition is not within the nurse practitioner’s scope of practice or the individual nurse practitioner’s competence.
- Advocating for clients in relation to therapeutic interventions, health care access, the health care system and policy decisions that affect health and quality of life.
- Collaborating with members of the health care team to provide and promote interprofessional client-centered care at the individual, organizational and system levels.
- Contributing to a safe, inclusive environment for all through compliance with patient and staff safety policies and procedures.
What must you have?
- Master’s Degree in Nursing
- Current certificate of competence from the College of Nurses of Ontario as an RN (EC) - Nurse Practitioner-Primary Health Care (PHCNP) or Nurse Practitioner-Adult (NP-Adult)
- Minimum of 5 years of recent clinical experience as a nurse practitioner required.
- 4 to 7 years’ experience working with the geriatric population required.
- National certification in Medical/Surgical or Gerontology Nursing from the Canadian Nurses Association required.

- Demonstrated knowledge of planning, developing and implementing education initiatives (health care providers/community)

- Provides comprehensive health services encompassing:
- Identification and management of chronic stable conditions.

- Support services.
- Advanced nursing assessment skills and sound knowledge of clinical therapeutics.
- Effective patient/family counseling skills.
- Demonstrated commitment to continuing professional development.
- Excellent computer literacy including knowledge of Excel.
- Excellent documentation skills and data collection.
- Valid driver’s license and availability of a car.
- Demonstrated good attendance record with the ability to maintain this same standard.
- Demonstrates a commitment to a healthy and safe workplace for self and others (staff, patients, families, etc.) by taking all reasonable precautions and working in compliance with hospital related policies, health and safety legislation and best practices and completing relevant mandatory education as required.
- Proven attendance record.
- Ability to perform the essential duties of the job.

Preferably, your profile also includes:
- National certification in Emergency and / or Gerontology Nursing from the Canadian Nurses Association.
- Membership in Registered Nurses’ Association of Ontario and Nurse Practitioner Association of Ontario.
- Experience in Case Management preferred.
