The Referrals Coordinator is primarily responsible for providing support for the timely and efficient processing and coordination of referrals to Inpatient Services. The incumbent may also be responsible for providing operational support for ongoing quality improvement initiatives act as a liaison with our acute care partners.

 

Summary of Duties

  1. Provide daily operational support for administrative staff in the coordination of referrals and bed management.
  2. Provide operational support for the program with a focus on implementing new processes, improving on established processes (e.g. referrals, intakes, scheduling), and improving communication.
  3. Liaise with acute care partners to facilitate the efficient and timely provision of care and to maintain/improve program responsiveness to stakeholder requirements.
  4. Foster and maintain positive relationships with referring healthcare care facilities to ensure a quality continuum of care.
  5. Facilitate the identification, implementation, and monitoring of continuous improvement initiatives related to key operational processes.
  6. Participate in program development and assist in the development of new business opportunities related to Inpatient Services.
  7. Conduct and analyze referral data reports from different referral sources and contribute to ongoing process improvement
  8. Other reasonable duties as assigned.
  9. Be aware of conditions and measures that may affect the safety of employees, patients, Sisters, students, volunteers, physicians and visitors including but not limited to: compliance with the safety standards of the Hospital and applicable legislation.
  10. Conduct their work in a safe manner; prevention, identification and reporting of safety issues and problems; taking corrective action, where possible; attendance at safety meetings / training / updates in accordance with the Hospital’s standards.
  11. May have access to confidential information related to Labour Relations.

 

Qualifications/Skills

  1. Bachelor’s degree in a health sciences or related field.
  2. Two years relevant experience within a health related setting.
  3. Current license and member in good standing with an affiliated professional college
  4. Excellent communication and organizational skills
  5. Excellent leadership skills
  6. Ability to facilitate cooperative working relationships with multiple internal and external contacts
  7. Proficiency in the use of MS Office software products and other associated programs.
  8. Evidence of good attendance, a satisfactory performance history and a discipline-free record required.
