Our Occupational Therapy program supports clients in achieving their goals to enhance their quality of life, maximize their safety, and improve their independence at home.\u00A0
VON Occupational Therapist (OT) helps clients of all ages develop, recover, improve and maintain the skills needed for daily living and working. \u00A0
They provide in-home therapy to adults after injury, surgery, or illness, promoting independence in daily tasks. They recommend equipment or home adaptations and work with them to help them return to leisure and work activities.\u00A0
The OT also collaborates with families, caregivers, and healthcare professionals, offering client-centered therapy using evidence-based practices.

What you\u2019ll be doing:
  • Demonstrating Occupational Therapy standards and guidelines for evidence-based practice to assess, plan, and evaluate progress (for potential discharge).
  • Assessing client\u2019s functional abilities in home/community, including Activities of Daily Living (ADL), equipment needs and cognitive function.
  • Considering holistic client needs impacting independence in home/community environment.
  • Developing a client-centered plan with the client/family and health care team (i.e. Care Coordinator, Physiotherapist, Personal Support Worker (PSW), Nursing, etc.).
  • Providing client/family education and funding assistance to obtain equipment and/or home/vehicle modifications.
  • Monitoring client progress toward therapy goals and adjust care plans based on client outcomes.
  • Recording client outcomes and care plans in the electronic client chart following the funder and College of Occupational Therapists of Ontario (COTO) guidelines.
  • Communicating with funding agency regarding client outcomes and status changes.
  • Effectively managing client caseload and updates care plans based on information from the client, family, and any other healthcare or educational professionals involved.
  • Utilizing verbal and non-verbal communication skills effectively with the client as required.
  • Demonstrating professional code of conduct and communication with all health team members as per VON guidelines and policy.

What we need from you: \u00A0
  • Masters in Occupational Therapy (or degree prepared occupational therapy education prior to 2008).
  • Valid registration with COTO and associated liability insurance.
  • Membership with the Ontario Society of Occupational Therapists is preferred.
  • Experience in caring for patients with varying diagnoses across the lifespan (stroke, Traumatic Brain Injury, falls, dementia, Parkinson\u2019s Disease, Chronic Lung Disease, Congestive Heart Failure, Obsessive Compulsive Disorder, developmental delay, learning disability, etc.),
  • Experience with patients who have musculoskeletal challenges an asset.
  • Capable of using computer software on phones or tablets and proficient in using technology to document client outcomes.
  • Excellent time management skills to balance client visits, paperwork and urgency of referrals.
  • Must have a personal vehicle and possess a current driver\u2019s license and proof of vehicle insurance.
  • Willingness to travel regularly during the day to support sites and attend meetings.
  • A current and original copy of a satisfactory Criminal Records Check is required, and a Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.
  • Must be able to wear Personal Protective Equipment (PPE).
