Location(s): Hybrid remote opportunity, work locations will vary, and travel within the service area is required. You may work from home and/or the VON office when not delivering services.
- Assesses and evaluates patient progress and reviews patient outcomes\u00A0
- Demonstrates knowledge, skills and attitudes as outlined in the Essential Competency Profile for Physiotherapists that provides a safe and evidence informed practice.\u00A0
- Consults with the patient to obtain information about their health, associated history, previous health interventions and outcomes.
- Collects assessment information relevant to client\u2019s needs and physiotherapy practice.
- Analyzes assessment findings to establish a physiotherapy diagnosis and prognosis.
- Develops and recommends a plan of care.
- Modifies the patient\u2019s care plan as required based on patient outcomes.
- Documents patient outcomes and current care plan\u00A0
- Communicates with interdisciplinary team as required regarding patient outcomes and status changes (for example most responsible physician)..
- Collaborates with the interdisciplinary team to ensure patient\u2019s care needs are met and therapy goals are progressing to meet patient individual goals.
- Utilizes verbal and non-verbal communication skills effectively with the patient/family as required.
- Demonstrates professional code of conduct / communication with all members of the health team as per VON guidelines and policy.
- Masters in Physiotherapy (or degree prepared physiotherapy education).
- Valid registered with the Nova Scotia College of Physiotherapy and associated liability insurance
- Member with the Nova Scotia Physiotherapy Association (preferred).\u00A0
- 2-5 years\u2019 experience in a health care environment preferred (i.e. long term care, acute medicine, rehab unit, rehab clinic).
- Recently demonstrated knowledge of stroke best practices in their practice (preferred).
- Recently demonstrated knowledge of musculoskeletal (i.e. arthroplasty) best practices (preferred).
- Trained in chronic disease management\u00A0
- Demonstrates knowledge of community resources preferred.
- Experience with patients who have musculoskeletal (i.e. hip and knees) challenges an asset.
- Demonstrates an ability to utilize computer software program(s) on a hand held device (i.e. phone or tablet).
- Demonstrates proficiency in the use of technology for documentation of patient outcomes.
- Ability to communicate well, both written and verbal.
- Ability to work interdependently and collaboratively with a multidisciplinary team.
- Ability to adjust to working in different social and economic environments.\u00A0
- Ability to respond to changes in patient caseloads and care needs. \u00A0
- Must have personal vehicle and possess both a current driver\u2019s license and proof of vehicle insurance
- A current and original copy of a satisfactory Criminal Records Check is required.
- A Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
- The use of Personal Protective Equipment (PPE) may be required
Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
