The Palliative Home Care program is an inter-professional team of health care professionals aimed at providing excellence in palliative care to individuals and families in the community setting. The program operationalizes the values inherent within Patient & Family Centered Care, in alignment with the AHS values. The Care Manager position is a leadership role within the Palliative Home Care program; and reports directly to the Area Manager of Palliative Home Care. This role supports an inter-professional team of mobile professionals and support staff in the community; ensuring client care and services are delivered using a client centered approach. The provision of staff support is fundamental aspect of this role. Strong communication and mentoring skills facilitate the internal functions of this role; and promote and maintain external partnerships, which are essential in the delivery of care. The Care Manager works collaboratively with Palliative Home Care, Integrated Home Care and the Palliative/End of Life portfolio leadership team to ensure program-wide objectives are met, including participating in committees, initiatives, program development, and projects.

Baccalaureate Degree in Healthcare Field plus appropriate licenses and designations. Minimum of 2 years in a similar leadership role ideally in an organization of significant size, complexity and diversity (healthcare experience required); preferably in a community based care setting. Proven problem-solving and conflict resolution skills are required. Proven record of strong interpersonal and communication skills are required. Demonstrated ability to collaborate and negotiate with a variety of internal and enteral stakeholders is necessary, Knowledge of change management and conflict resolution frameworks are necessary. Demonstrated knowledge and ability to coach and mentor is required. A demonstrated clear pattern of professional and personal development with training/courses or certificates in leadership development, change leadership, effective communication, team building, quality improvement and budget management preferred. Valid Driver's License and access to a reliable motor vehicle for day to day travel is required. Physical ability to perform job functions is required.

Demonstrated leadership skills, including demonstrated ability and comfort with decision making responsibilities, coaching and teaching, and the ability to inspire and build confidence in others. Ability to manage human, financial and physical resources within an operating environment. Proven ability to foster partnerships and to achieve organizations goals within and organization and ideally managing within a unionized sector. A demonstrated clear pattern of professional and personal development. Encourages innovative processes and supports the culture and practices of Patient and Family Centered Care.

Master's Degree Preferred. Knowledge related to Palliative Care clinical practices is preferred.
