Description
Working collaboratively and as part of a multidisciplinary team the Social Worker II is involved in the provision of Social Work services to patients and families. The RAH Social Work department is a fast paced exciting facility that provides opportunities to work, learn and grow within an interdisciplinary team. The work may include psychosocial assessment, crisis intervention, complex discharge planning, financial assessment, capacity assessment processes, education, treatment, advocacy, resource finding, consultation, making appropriate referrals both internal and external to the hospital and liaising with community agencies/programs. The Royal Alexandra Hospital Social Work Department strives to provide excellent patient centered care through ongoing mentorship, education and implementation of evidence informed and best practice strategies. The Social Work Department provides excellent learning opportunities through professional practice lead events as well supporting ongoing professional growth and quality improvement initiatives. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
A Bachelor or Master of Social Work degree (BSW) is required. Active registration with the Alberta College of Social Workers (ACSW) is required. Greater than 1 year experience in an inpatient adult acute care setting is required. Demonstration of strong critical thinking and problem solving is required. Experience working successfully in a collaborative interdisciplinary team setting is required. Experience with capacity assessment processes, financial assessments and complex placement processes is an asset. The candidate will have proven communication skills to work cooperatively and effectively with the health care team members, patients and their families in a fast paced environment. Strong professional judgment, initiative, and effective time management skills are necessary.
Knowledge and ability to use computer applications such as Microsoft Office (Excel, Word and Outlook). Experience with Connect Care is preferred. Experience with capacity assessment processes, financial assessments and complex placement processes is an asset.
