Foothills Medical Centre (FMC) is an Acute Care hospital with over 1,100 beds located on the west side of Calgary, Alberta. It is the Trauma Centre for Southern Alberta, Eastern British Columbia and Western Saskatchewan, and provides the majority of In-Patient Neurological services in Calgary. Additionally, FMC has a close partnership with the University of Calgary medical teaching program and the Libin Cardiovascular Institute. The Allied Health department at FMC consists of Physiotherapy, Occupational Therapy, Social Work, Spiritual Care, Speech Language Pathology, Recreation Therapy, Therapy Assistants, Psychology and Administrative staff. Allied Health staff work as integral members of the interdisciplinary team to provide patient and family-centered care throughout the hospital. The present caseload for this position is within the Medicine/Surgery team, with the potential for coverage on any unit/service as required. The caseload for all positions within the Allied Health department may be subject to change based on operational considerations. Under the general direction of management and the Clinical Leadership team, you will perform duties primarily related to assessment, treatment, education and management of patients across a variety of clinical areas. To be successful in this role, you will possess an understanding of how patients and family are at the center of care strategies to build inclusive environments that reflect the needs and supports required for the populations that we serve (ethno-cultural, equity-seeking, racialized, faith-based, LGBTQ2S+, vulnerable, marginalized, persons with disabilities, and persons of all age groups).

As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Minimum of a Bachelors Degree from a recognized Social Work education program required. BLS for Health Care Providers Level C CPR certification issued within the last 12 months prior to commencement of employment.

Minimum one year of experience working in adult acute care is preferred. Experience with complex discharge planning within an acute care setting is preferred.
