This is an exciting opportunity to be a member of the Assertive Community Treatment Team (ACT). Reporting to the Care Manager, the Social Worker will provide assertive, client focused, culturally appropriate, recovery-oriented care to adults experiencing severe and persistent mental illness, addictions and houselessness. ACT works primarily with individuals whose illness presents a barrier to engagement in treatment. Clinical services are most often delivered through outreach services in the community, and in partnership and collaboration with clients, families, other providers, inner city agencies, and community programs. ACT also has a partnership with Homeward Trust Edmonton. As part of the multi-disciplinary team, the successful candidate will be responsible for supporting clients who require assistance in initiating, obtaining, and maintaining housing resources and supports. Building and maintaining relationships with landlords is also an integral part of this role. This position has transitioned to Recovery Alberta effective September 1, 2024. As a Social Worker I, you will be a member of a multi- disciplinary team that provides mental health services to individuals, families and/or groups at a community based facility. You will be responsible for the initial screening of patients by gathering and documenting client information, basic assessment and providing support to clients and their families' short-term needs. You will liaise with and refer clients to appropriate community and government services and support, along with participating in case conferences and team meetings.

Completion of diploma in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).

Minimum of 2 years of relevant addiction and mental health experience, preferably in a community setting. Demonstrated ability to work well in a team and seek consultation when necessary. Ability to ambulate, climb stairs, and move quickly without limitations in a variety of environments. Possession of a valid Class 5 driver’s license. Experience and knowledge of the housing first, harm reduction philosophies of care.

Demonstrated ability to work well with a range of community and agency stakeholders as well as clients and families. Working knowledge of addiction and mental health community resources. Experience working with individuals who have barriers to housing.
