Description
The Centre for Addiction and Mental Health (CAMH) is seeking a full-time, contract (until March 31, 2025) Advanced Practice Clinical Leader (APCL) for the Professional Practice Office. Reporting to the Director of Interprofessional Practice, the APCL will provide professional practice leadership across the organization in supporting the advancement of measurement based care. You will co-design resources for clinical programs to support the development, implementation, and sustainability of measurement-based care and care pathways across the organization. You will act as a liaison between the technical team, project management team, standards and measurement advisory committee and non-physician clinicians across the organization in the development of care pathways and workflows to advance patient outcomes.
The APCL will also be responsible for initiating and leading a Community of Practice with CAMH APCLs and clinicians to support measurement based care planning, implementation and problem solving. The APCL will work alongside the project team and technical team to support design and testing of technical solutions developed via this project. Further, this role will involve consultation with clinicians, patients and family members on the planning surrounding measurement based care at CAMH. The APCL will also conduct presentations both internally and externally, and participate in authoring publications based on the outcomes of this project.
You will participate on internal and external committees and working groups; and serve as a clinical resource and subject matter expert in strategic planning activities related to measurement based care. The successful candidate will possess a Master’s Degree in Nursing or relevant field and be a member of good standing in the associated regulatory College. In addition, 5 years clinical experience involving clients with mental health, addiction and forensic issues is required. Eligibility for cross-appointment with the corresponding University of Toronto Faculty is also required. The successful candidate will have the ability to provide leadership in practice development; the ability to challenge and motivate others; commitment to enhancing academic and research endeavors; experience in leading change; monitoring and measuring outcomes; and the ability to provide advanced clinical care in a culturally diverse environment. You will have experience with integrated care pathways, measurement based care as well as clinical leadership and staff engagement expertise. You must possess excellent interpersonal, communication, negotiation, teaching and computer skills. Bilingualism (French/English) and/or proficiency in a second language is considered an asset.
