REPORTING TO: Director of Care/Assistant Director of Care

SUMMARY OF DUTIES:

Under the direction of the Director of Care, or designate, the Clinical Manager ensures proper provision of nursing care for residents within a resident home area(s), which includes but not limited to: planning, coordination, implementation, evaluation of care as prescribed by a residents physician and care plan, provides support to nursing team under their direction, liaises with families and residents to address needs and concerns, supports performance of those working within the resident home area(s), leads and/or participates on committees or working groups, communicates information to direct reports from the leadership team, supports home/organizational specific programs and policies.

May be responsible for the general operation of the facility in the absence of the designated responsibility person, with the support and guidance of the Admin-on-call.

QUALIFICATIONS:

Current registration status with the Manitoba Nurses Association of Registered Nurses or the Registered Psychiatric Nurses’ Association of Manitoba. [Nursing Administration certificate, Bachelor of Nursing Degree or equivalent is preferred.] Some supervisory experience also preferred.

Must be knowledgeable in principles and philosophy of long term care;

Ability to supervise and direct the work and learning of others.

Ability to work independently with minimal supervision and complete assignments in a timely manner

Ability to communicate effectively with other staff, residents, families and the public and community resource personnel

Complete all education as required by Extendicare or by the regulatory body


DUTIES AND RESPONSIBILITIES:

Oversees the care and services within a resident home area(s) Assists the Director of Care and Assistant Director of Care in the development and maintenance of care standards, facility policy and procedure. Establishes and implements annual goals and objectives for the Nursing Department in accordance with facility goals and objectives. Participates in inter-disciplinary conference within the prescribed timelines and provides support in completion of the meeting and documents. May notify physicians, residents, families and Director of Care of any changes in a resident’s condition. Ensures arrangements for diagnostic procedures and examinations are made in accordance with physician orders. Ensures resident health care records are an informative, accurate and current description of the health status of residents by adhering to charting policies and procedures. Assigns staff and schedules work assignments in accordance with staff complements including:

Planning daily work assignments in relation to resident needs;

Assigning duties to nursing personnel by maintaining group lists, bath lists, cleaning duties, special assignments, etc.

Replaces or requests replacement of absent care staff to ensure staffing compliment for all shifts, adhering to facility/company policies and procedures. May facilitate rounds with physicians, transcribes physician orders and incorporates them into the resident care program; may also be done with the support of clerks. Supports orientation of new staff, as required, including the ongoing instruction and supervision of all nursing staff. Promotes ongoing staff education by participating in formal and informal teaching programs for all members of the health care team as well as health teaching to residents and families as appropriate. Regularly evaluates care staff with feedback from clinical team and support form DOC and ADOC.. Assists in the completion and presents an annual employee appraisal to all unit staff including plan of action for employee development. Supports the admission of new residents and orientates new residents and their families to the facility as per the admission procedure. Assists in the development of and ongoing review of individualized care plans. Monitors completions of MDS assessments and quarterly care planning. Maintains proficiency in computer skills related to MDS completion. Monitors the use and maintenance of supplies and equipment on the unit. Requisitions supplies and equipment when necessary. Practices effective cost control. In the absence of the Administrative personnel assumes administrative responsibility for the entire building when in charge on weekends, nights, and after hours. Contacts appropriate management personnel if concerns, problems or emergencies arise. Performs all duties in accordance with the written policies and procedures of Extendicare, the facility and the Nursing Department. Participates in the evaluation of care and/or service provided by the Nursing Department by developing, implementing and evaluating activities under the Quality Assurance Program. Performs all duties in a safe and efficient manner, maintains work area in a clean, orderly and safe condition. Follows manufacturers’ instructions in the operation and use of all equipment and chemicals. Alert to potential health and safety hazards and takes appropriate action. Reports unsafe and hazardous working conditions and any equipment malfunctions to Administration. Knowledgeable of facility procedures for fire and environmental safety which includes completing building rounds as per schedule

Performs duties in accordance with prescribed sanitation and infection control procedures. Adheres to but not limited to “Standards of Employee Conduct,” “Commitment to Residents,” “ Bill of Rights”, and “Extendicare Quality Care Standards”, , Extendicare Mission, Vision and Values, and Manitoba Health Standards.

Works closely and cooperates with other staff and residents. Maintains good working relationships with other departments.

Updating Admin/DoC/ADoC of emerging concerns that pose a risk to the Home. Performs other related duties as assigned.
