Job Description

REPORTING TO: Director of Care, Assistant Director of Care, Unit Manager, Unit Coordinator

SUMMARY OF DUTIES:

Under the direction of the Director of Care or designate and supervised by the primary care nurses, the Health Care Aide, provides direct personal care that support activities of daily living to fulfill the physical, intellectual, emotional, social, and spiritual needs of residents.

QUALIFICATIONS:

Health Care Aide certificate received from a recognized college, or approved nursing equivalent Combination of medical training and experience will also be considered. For Uncertified HCA, high school diploma is required with some experience in a medical field very beneficial Ability to read, write and speak English as well as to understand verbal and written instructions in English.

  • Ability to work independently with minimal supervision and completes assignments in a timely manner.
  • Ability to communicate with other staff, residents, families and the public.

DUTIES AND RESPONSIBILITIES:

Monitors, reports changes in the resident’s physical or mental condition to the primary care nurse.

Responds to resident call bells and provides appropriate assistance, in a timely manner, as required.

Maintains resident living areas in clean, organized and in-line with residents’ desires, as required.

Participates in care conferences/IDTC to support communication between all parties and families/residents.

Completes all documentation to ensure accuracy and timely in the tool prescribed by their supervisor. Encourages, supports, and participates, as directed, in the facility activity programs, including transporting residents to Activity programs.

Collaborates with other colleagues within the home regarding supply needs, equipment needs/repairs Performs all duties in accordance with the written policies and procedures of Extendicare, the facility and the Nursing Department.

Participates in evaluating care and/or services provided by the Nursing Department to residents under the Quality Assurance Program.

Performs all duties in a safe and efficient manner and maintains work area in a clean, orderly, and safe condition, including reporting any unsafe and hazardous working conditions and any equipment malfunctions to the supervisor.

Adheres to “Standards of Employee Conduct”, “Commitment to Residents”, “Bill of Rights” and “Extendicare Quality Care Standards” , Manitoba Health Standards and Mission, Vision and Values.

Performs other related duties as assigned.

Mentally and physically able to perform duties as outlined which includes ability to maintain regular attendance in addition to work in a very dynamic environment.Complete all education as required by Extendicare Provides care inline with resident wishes and care plan with active resident involvement to enhance their quality of life, in all areas of activities daily living which include bathing, dressing, meal assistance, attending recreational programs/events, oral care, grooming. All care and interactions are to respect residents’ rights to dignity and privacy and in a courteous and respectful manner.
