Clinical Nurse Specialist

The clinical nurse specialist (CNS) will utilize his/her advanced knowledge of the stroke patient population and program development skills to work in a collaborative model of care. The collaborative model of care will support the transition of stroke patients across the continuum and to the community through capacity building, collaboration, and partnerships. The CNS will work in partnership with the physicians and other members of the inter-professional team.  The position will report jointly to the program director of regional rehabilitation and integrated stroke and the chief of interprofessional practice.


Qualifications
  1. Current Certification of Registration with the College of Nurses of Ontario as a Registered Nurse.
  2. Master’s Degree in Nursing
  3. Minimum of five (5) years recent clinical acute care experience; preference supporting a specialized population of acute stroke patients, neurology, neurosurgery or other relevant experience will be considered.
  4. Demonstrated ability to contribute to program design and evaluation, including the identification of outcome indicators.
  5. Demonstrated excellent interpersonal and communication skills, and competency to work as a member of an interprofessional team.
  6. Demonstrated ability for innovation, independent action initiative, problem solving, organization and prioritization.
  7. Demonstrated commitment to patient and family-centered care.
  8. Demonstrated commitment to ongoing professional development.
