Job Description:
Under the direction of the Manager, and as part of an interdisciplinary team, the Assessor Coordinator - Degree - Senior is responsible to provide a complex range of screening, assessment, treatment and case coordination services to the community mental health and addictions population seen at the Urgent Care Center . The incumbent will provide consultation and coordination of service including prioritizing and referring to community agencies . Also provides evaluation of clinical interventions, supervision and professional and public education regarding mental health issues and resources . The Assessor Coordinator - Degree - Senior will provide leadership and clinical expertise in the development and maintenance of community mental health and addiction treatment programs . The Assessor Coordinator - Degree - Senior, along with community and the Urgent Care Center teams, provides crisis intervention, support, assessment and follow-up plans to clients in the community who are in crisis or require assistance. The Assessor Coordinator - Degree - Senior also assists in the successful stabilization and /or transition of acute patients from UCC to hospital/detox or alternatively to prevent admission/readmission by providing short term acute mental health/addiction services to those who present in crisis. Follow-up may include brief intervention/support, referral to appropriate community organizations/support networks or collaboration with existing supportive organizations. Prevention of ongoing or future crisis is a goal. Services are provided 24/7.

Education
- Recognized degree in nursing, social work or addiction counselling.
Experience
- Minimum of 2 years (postgraduate) work experience working within community mental health/addiction services.
- Post-graduate training or courses in psychiatric nursing is an asset.
Knowledge and Abilities
- Demonstrates a high degree of knowledge and skill in assessment, triage, treatment, case management, community outreach and crisis interventions.
- Knowledge of normal child and adolescent development and psychopathology. Knowledge of family systems, and models for therapeutic intervention strategies appropriate to the treatment of child, adolescent mental health diagnoses and family problems. Knowledge of interventions empirically validated or supported by best practice literature.
- Knowledge of a variety of mental health disorders and addiction and harm reduction strategies required to enable clients to sustain stability.
- Ability to identify, implement and evaluate areas of client difficulty across a wide range of clinical presentation and formulate evidence-based intervention.
- Ability to implement a variety of intervention strategies (individual or group) for individuals with mental health and addiction problems in a community-based setting, including but not limited to assessment, triage, treatment, case management.
- Knowledge and practical application of the Mental Health Services Act, the Family Services Act and the Health Information Privacy Act.
- Demonstrated cultural competency skills.
- Organizational skills, including the ability to maintain priorities while managing multiple and diverse demands.
- Demonstrates strong leadership abilities and the ability to work within a change management model. Interpersonal competence, including effective problem solving and conflict resolutions skills.
- Knowledge of community resources and demonstrated ability to develop and maintain consulting relationships within a multidisciplinary team, interagency programs and across the community of human services.
- Ability to work collaboratively as a member of a multidisciplinary team, and independently to manage complex and multiple demands.
- Sound communication skills - both written and oral. Ability to prepare reports in a clear, concise and timely manner and ability to provide public presentations and professional training.
- Effective problem solving, decision-making and conflict resolution skills.
- Ability to enter and retrieve data in a computer database system.
- Ability and willingness to travel and work evenings/weekends as required.
- Basic computer skills.

- Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. HSAS seniority within the Saskatchewan Health Authority will be considered under the Selection Criteria.
